Forest Farm Health Care Center is a licensed skilled nursing facility (SNF) that provides Short Term Rehab (physical, occupational & speech therapy) and long-term care services for up to 50 residents. Forest Farm is staffed on a 24-hour basis with qualified nurses and certified nursing assistants. The services of the medical director, assistant administrator, a registered dietician, a pharmacist's consultant, and a social worker are also available.

The activities program is creative and innovative, always responding to an individual's needs. The Rehabilitation Therapy Program is provided by Sundance Rehabilitation.

We realize that persons inquiring about health care facilities often have many questions. It is always desirable to have family members, friends, or prospective residents visit the facility. In order for Forest Farm to properly address the needs of potential residents, it is necessary to exchange certain information relating to a resident's personal needs and preferences.
