Background

Forest Farm Health Care Center has been providing services to the Aquidneck Island community since 1932. Located in a rural setting in Middletown, Rhode Island, this modern facility was constructed in 1975 and was designed to meet all state and federal standards for a health care facility. We are also pleased to announce the addition of the Forest Farm Assisted Living Center, a 50-unit assisted living facility that opened in 1999.

Since its inception, Forest Farm has continued to expand to meet the ever-changing health care needs of the community. The Adult Day Care, Assisted Living, and NewCare provide elders and their families quality support at different levels of care.

When the time comes for elders and their families to seek long-term care options, Forest Farm has the ability to initiate a care plan that will best suit a client's needs. Along with traditional family support, the many different service areas available can assist in making it a reassuring decision.

As you may know, the government has begun publicly reporting information that it claims relates to the quality of care provided at nursing homes in Rhode Island. Forest Farm Health Care Centre agrees that it is important for you to be informed about quality of care issues. We are concerned, however, that shortcomings in the methods used by the government to derive this information may leave you with incomplete and inaccurate information regarding the quality of services provided.
