ABOUT US

Forest Farm Health Care Center is a licensed skilled nursing facility (SNF) that provides Short Term Rehab (physical, occupational & speech therapy) and long-term care services for up to 50 residents. Forest Farm is staffed on a 24-hour basis with qualified nurses and certified nursing assistants. The services of the medical director, assistant administrator, a registered dietician, a pharmacist's consultant, and a social worker are also available.

The activities program is creative and innovative, always responding to an individual's needs. The Rehabilitation Therapy Program is provided by Sundance Rehabilitation.

We realize that persons inquiring about health care facilities often have many questions. It is always desirable to have family members, friends, or prospective residents visit the facility. In order for Forest Farm to properly address the needs of potential residents, it is necessary to exchange certain information relating to a resident's personal needs and preferences. We have found that the most opportune time to gather that information is prior to admission.

Forest Farm Rehabilitation

Forest Farm offers and is able to provide comprehensive services that may include physical, occupational, or speech therapies, depending on your specific needs. Clinical problems such as stroke, COPD, fractures, and post-surgical treatments are examples of problems some of our patients are admitted with.

We now have the Nintendo Wii Fit Sport, Fitness & Exercise program, an interactive computer excercise program!

We realize that decisions made by the Rehab Team have a great impact on your return home, so discharge planning is something that begins at the time of your admission. The decision to continue or to conclude therapy is based on various factors that include, but are not limited to:

 

  • Evaluation Results
  • Patient's Response to Treatment
  • Patient's Need for Skilled Therapy
  • Insurance Requirements

An Experienced and Dedicated Staff

The term skilled therapy is used to describe the type of care that is complicated enough to require the skills and expertise of a therapist or a licensed nurse versus maintenance which is routine exercise or assistance for daily activities which do not require skilled care. Most insurance companies will only pay for skilled therapy. As your treatment transitions into maintenance rather than skilled therapy, alternative plans can be developed to meet your needs. The rate of recovery differs for every individual; therefore you will want to speak with your therapists to get a better understanding of how you are progressing.

Some residents have insurance to help pay for their short-term rehabilitation. Insurance coverage, including some of the managed care coverage, has specific guidelines that must be met. We encourage you to review your particular plan's coverage.

For those residents who are anticipating a return home, the Social Services Department is available to discuss options with you, including various services that may be available. The Social Worker will also inform you of the Patient Care Conference (PCC) which you are invited to attend. It provides a resident and their family with opportunity to meet with the following departments: Nursing, Dietary, Social Services, Activities, and Administration. Your special needs will be discussed at that time. Of course, please remember that at any time our staff is available to answer any questions you may have.

Quality of Care Information

As you may know, the government has begun publicly reporting information that it claims relates to the quality of care provided at nursing homes in Rhode Island.  Forest Farm Health Care Centre agrees that it is important for you to be informed about quality of care issues.  We are concerned, however, that shortcomings in the methods used by the government to derive this information may leave you with incomplete and inaccurate information regarding the quality of services provided.

For example, the government methodology fails to adequately take into account the size of facilities.  A small, personalized facility such as Forest Farm Health Care Center (we have fewer than 63 beds, many of which are private rooms) does not have a large enough census to produce statistically meaningful results.  Accordingly, information in the government report that seems to imply a lower quality of care, may actually be a result of the statistical shortcomings of the methodology used.

A related problem is that some of the information reflects only certain assessments of Medicare patients.  In a facility like Forest Farm Health Care Center where the majority of resident' care is not paid for by Medicare, the small sample size used leads to inaccurate information regarding the quality of services.  For example, in a facility in which Medicare patients make up only 10% of the patient population, some of the government's information regarding the entire facility is derived from fewer than 10% of the facility's patients.  This is so because some of the government information comes only from Medicare patients and only from those Medicare patients who meet certain clinical criteria.  Therefore, the government's report about a facility like Forest Farm Health Care Center does not take into account important information regarding the majority of patients served by the facility.

Another problem with the report is that some of the information on which it is based relies on subjective judgments that are open to interpretation and vary from reporter to reporter.

We are proud of the quality of care that we provide at Forest Farm Health Care Center.  We do not believe that information reported by the government accurately or adequately reflects the high quality, personalized service that is the hallmark of Forest Farm Health Care Centre.  I encourage you to seek out complete and accurate information regarding quality of care issues.  This organization supports initiatives of Quality Improvement and utilizes data from more than just one source.  The most important way to make a decision regarding placement in long-term care is taking tours and asking about the facility.  Please feel free to call me if you have any questions.
